American Airlines has handed me a schedule change...departure time 7:00 AM. How busy is the airport at this time of day? Will I need the full 2 hour before check-in as recomended?

If you're not happy with the new time here's a trick. American has changed our flights several times. I think there is a rule if they change it XX amount of minutes (maybe 90?) you can request a different flight at no charge. We have even had it changed to a different day. Check it out, works for us all the time.
They change my up & coming January outbound from ORD to MBJ from a 8:50am to 7:25am departure...I'm OK with that.....last year they changed the return part of my trip to ORD from a nonstop to a stop in Ft Lauderdale without asking me....I don't do stops so got them on the horn & they said no more nonstops on Wednesdays anymore so we changed it to Tuesday...& it wasn't an easy thing to do...they had to refund the whole trip & start from scratch...ridiculous
0700 Sanster? Jeez..... Louise
Might as well pull an all nighter!

I thought the same thing. Your driver will likely want to pick you up by 3:30am. I would just stay up all night if I couldn't change the flight.
Actually, I'm surprised AA has flights that early out of MBJ. Usually, the planes flying to the US are ones that have flown in to Jamaica that day. At least that's the way the PHL-MBJ route works.
What happens if you pretend you didn't get the notification? You are on vacation and might not have internet access, right?
Where are you leaving from? They say if leaving from Negril to allow 2 hours for travel incase you have any delays. Depends on how much time you want to spend in the airport. It shouldn't be too busy on a Tuesday.
