I used it last month and did not fill out a paper form. Just checked and it is back up and working now.

After entering the required details, passengers must declare that all information provided is correct and true. There is a processing fee, which can be paid with a credit or debit card.
Once this is done, the traveler can submit their online C5 form to make their customs declaration.

Filled mine out this morning and it is working well. Just be cautious, the jamaicac5.com link above is a scam/ fraud
