And only use that one site. If you google Jamaica C5 Form you might be taken to some sites that ask for "the entry payment". Those sites are scams. Any site that asks for money to enter Jamaica or requires a service charge, is a scam.

My lodging does not have an official street number. Like most places it just says their address is simply Norman Manley Blvd. Does this cause an issue on C5 or with C/I checkpoint?

Hmm, Leaving on March 30 so went online to fill this out but the charter airline we are booked on is not among the choices available for selecting which airline we are arriving on.

Tempted to select "Private Flight" but that's not accurate...

I think charters are technically considered private flights. When we were bringing students down for Spring Break from '89 to '94, our charters were considered private flights.

But let me do some calling to find out for sure.
