Be careful of the fraud sites that are out there. If they ask for money when you are filling out the form, you are being scammed and they also will have your credit card number.
We flew Delta in February - it was part of their fly ready documentation to fill out the form

You have to fill out the C5 form online. We didn’t find that out until we were in passport control. I recommend filling it out ahead of time.
