Jamaica Marriage Certificate/License and new passport question

So married in JA several years ago. Passports are expiring this fall so I am getting ready to send in our "stuff"
One of my needed items is my marriage license (name change).
Just realized that the "license" has no signatures of our people that signed the "register". The register was given to us on the wedding day.
License came in the mail about eight weeks later.

What do I send in for my passport - both items???

Hi! I was married in Jamaica as well in 2010. =) All you should have to send in is the marriage license along with your name change forms etc.. for the passport. It should ask you somewhere on there what your old name was and what your new name should be.
