I have been running on the beach for over 30years. Very hard to run on the beach south of Bar b Bar sand is too deep. Running past Tonys Hut some time no beach. Most times no beach by Treehouse bar. Cars on road too scary for me to run there.

Its safe. I walk the beach early in the mornings around 530-6am. I see runners all the time running along side the road; not sure how safe that is though. Alot of people are out that time, either going to work or preparing for the day.
Keep an eye out for the sea horses.

Like I said... the road is perfectly safe as long as you apply common running rules & use common sense AND run before there's much traffic. I figure w/ all the time I've spent in Negril & run along the road I'm looking at over 2 years of running on the road. Cars closer than comfort for me only a couple of times & that was when cars were 1.) passing (coming from behind & I heard them) 2.) passing on the shoulder - only one time. That was the only close call. Not bad for well over 700 runs along the road.
Kaycee... As I've said, road is perfectly safe as long as you get out early enough & pay attention.
For those of you who choose the beach... if it works, good for you. Too risky for me.
I love to walk on the beach in the morning. I walk sand & road.Do CoCo to Hedo and back. Once I walked on the slanted sand alot and I did get sore shin splints though....Walk higher on the sand now where it's more level...
